Evolve Anti-Aging & Wellness Center Inc ("Company," "we," "our," or "us") is committed to protecting the privacy and security of our patients' protected health information (PHI).        Right to Restrict Use and Disclosure: You can request that we limit how we use or share your PHI for treatment, payment, or healthcare operations. We are not required to agree unless the request pertains to a service that was paid for out-of-pocket in full.

       Right to an Accounting of Disclosures: You can request a list of instances in which we have shared your health information for six years prior to your request, excluding disclosures for treatment, payment, and healthcare operations.

       Right to Obtain a Copy of this Notice: You can ask for a paper copy of this Privacy Policy at any time.

       Right to Appoint a Representative: If you have granted someone medical power of attorney or if someone is your legal guardian, they can act on your behalf concerning your health information.

       Right to File a Complaint: If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

       Additional Rights Under the CCPA: California residents have additional rights regarding their personal information:
